Provider Handbook
The Provider Handbook can be downloaded in its entirety in the zipped version,
or you can download only the chapter in which you are interested. This facilitates
the update of any printed versions you may use in your organization. The
files below will always contain the most recent version of the Provider Handbook.
The version number will be updated with each revision, and the effective
date published on the cover sheet for each chapter. If you would like to
