Customer information authorization form
If you require usage history or other information on a customer prior to enrolling
that customer in the Texas Choice Program, you must submit a signed authorization
from the customer to obtain any such information. The files attached below
are provided for this purpose.
Once completed and signed by the customer or its legal
representative, fax or email the completed form to our Billing
and Account Operations department at:
