The Provider handbook has been created to provide a detailed overview of Customer
Choice implementation to help facilitate your entry in the market and to
keep you abreast of any changes in operations. Directly below you will find
a section where updates to the Provider handbook may be found. We will provide
a red-lined version of the revised chapter(s) to better aid you in identifying
the changes to the Handbook. If you have any questions about the content
of the Handbook, please contact Provider support.
Provider handbook
The Provider handbook can be downloaded in its entirety in the zipped version,
or you can download only the chapter you are interested in. This is to help
to facilitate the update of any printed versions you may use in your organization.
The files below will always contain the most recent version of the Provider
handbook. The version number will be updated with each revision, and the
effective date published on the cover sheet for each chapter. If you would
like to share any comments or suggestions about the content of the Provider
